Overview

MHA in Ulster County, Inc is seeking a full-time (37.5 hours per week) Program Director to oversee and supervise all aspects of the Children’s Health Home Care Management Program. The purpose of the Health Home Care Management Program is to outreach, enroll and assist all youth who are eligible for Health Home Care Management Program.The program works with children, families, and service providers to create a comprehensive Plan of Health Care, identify additional services as necessary, and coordinate the continuing involvement of families and service providers in the execution of the plan. 

Responsibilities

 

In addition, this position will supervise the Home & Community Based Services Waiver (HCBWS) Program Care Coordination through its transition into a Children’s Health Home Program.  HCBWS provides care coordination of services for intensive in home, crisis response, family support and respite. The program links children and families to services identified in the service plans that will provide the support they need.

 

The Program Director will be responsible for all aspects of maintaining the programs, which includes all administrative activities, staff supervision, review of service plan documentation and records, Medicaid billing and fiscal management responsibilities. This position is also responsible for the enrollment of potential participants, coordination of the intake and discharge from the program, working with local, county and state agencies as needed, and overseeing the Quality Assurance of the program, Case Record Review, Utilization Review, and Incident Reporting according to regulations. 

 

Qualifications

Required Qualifications Include:

  • Qualifications include Licensure in a Human Services field, such as LMHC, LMSW, LCSW, or Registered Nurse; AND 3 years of relevant experience and 3 years of supervisory experience required.

  • Knowledge of both behavioral health and physical health issues is a must.

  • Knowledge of Ulster County behavioral health, physical health, and community services is vital.

  • Must have a clean, valid NYS driver's license with 3 years driving experience.
